Information you choose to submit
The appointment form asks for a caregiver name, phone number, optional email, preferred contact time and broad reason for contact. Do not send symptoms, reports, identity documents, payment details or other sensitive medical information. The production form routes to server@milestoneskochi.com. Local development validates the form without sending mail.
Always-on first-party base statistics
Production pages request the site’s shared /collect/ endpoint once on page load. This base event is independent of cookie acceptance and is designed to be cookieless. It may process the referring page, page URL through the HTTP referrer, IP/network information present in the request, browser language, local timestamp and timezone for security, aggregate traffic and error diagnosis. The production collector stores aggregate and limited request records in the site operator’s infrastructure. Loopback development traffic is excluded.
Cookies and optional features
The base statistics request does not require a cookie. The site does not currently load advertising, personalised tracking, social pixels or third-party analytics. If optional advertising or personalisation is added, the consent interface and this notice must be updated before activation. Essential session storage may be used to protect the appointment form from cross-site submission.
Logs, retention and security
Hosting systems may keep standard security and access logs. Appointment email retention depends on the operational mailbox and must be governed by the clinic’s confirmed retention practice before launch. No Internet transmission is completely secure. Access should be limited to people who need it to handle the request.
Your choices
You may call instead of using the form. You can ask about an appointment request, correction or deletion using the site contact address, subject to legal and operational needs. This notice does not claim rights or jurisdiction that have not been confirmed.
Children
The website is directed to parents and caregivers, not to children submitting their own personal information. A caregiver should make contact on a child’s behalf.
