What this guide helps with

Use it to prepare questions, understand the service pathway and decide what to confirm with a provider.

What to know

Call the provider to confirm the appointment, branch, professional type and records requested. Keep vaccination records, relevant school observations, prior reports and a concise timeline available for the actual provider. Write down the main concern, examples from more than one setting and the outcome you hope to understand. Do not upload medical records to this directory. If the child is acutely unwell or a caregiver believes there is an emergency, seek urgent in-person help instead of waiting for an online response.

Questions to take to the provider

  • Does this specific branch currently provide the service?
  • Which professional will assess or support the child, and what are their credentials?
  • Is a referral, appointment or prior report required?
  • What should the family bring, and how will the next step be explained?

Limitations

This guide cannot determine which professional, assessment or treatment is appropriate for an individual child. Confirm branch services and current availability directly with the provider.
